Scarring alopecia (Cicatricial alopecia )

Scarring alopecia, also known as cicatricial alopecia is a kind of  hair fall problems that may be diagnosed in up to 4% operson who have hair loss problems. It can be seen  worldwide in otherwise healthy males and females  of all ages.

Every specific diagnosis within this subject is not seen so much, but some examples include examination of  cellulitis, eosinophilic pustular folliculitis, follicular degeneration problem (generally known "hot comb" alopecia), folliculitis decalvans, lichen planopilaris, and pseudopelade of Brocq, to name a few. Scarring alopecia can ocur as a part of a much larger situation such as chronic lupus erythematosus, where some organs of the body have risk to face.

While there are a lot of types  scarring alopecia, gnerally theme is a potentially permanent and irreversible harmful effects of hair follicles and their replacement with scary part.

Most types of scarring alopecia first seen as small patches of hair loss that may increase  when the time passed. Sometimes, the hair loss is unvisible, without noticeable symptoms, and may go unnoticed for a long time. In other symptoms are  severe itching, burning on the scalp, and pain, and is quickly progressive.

The scarring alopecia patches generally look a few different from alopecia areata in that the edges of the bald patches seen  more "ragged." The harmful effect of the hair follicle consists below the skin surface so there may not be so much to actually aware on the scalp skin surface other than patchy hair fall. Problem areas may be smooth and clean, or may blushed, high or low pigmentation, or may have increased blisters with fluids or pus coming from the affected part.

These symptoms may help to understand, but it is not easy to diagnose a scarring alopecia just from the pattern of the hair fall and the nature of the scalp skin. Generally when someone think that they have scarring alopecia , one or more skin biopsies are done to  diagnosis and help to understand the particular form of scarring alopecia. A small biopsy of 2 to 4 mm in diameter is done and searched under a microscope. Doctor will look for destruction of the hair follicles, scary area deep in the skin, and the presence and location of inflammation in connection to the hair follicles.


Scarring alopecia  generally burns out. The bald patches stop increasing and any inflammation, itching, burning, blushing or pain goes away. At the end, another skin biopsy generally shows no inflammation around hair follicles. Bald parts usually have not so much hair follicles. At least those at the periphery of a bald area, are not fully destroyed and they can ocur again, but mostly, all that is left are just a few longitudinal scars deep in the skin to explain where the hair follicles occured before.

Cure Choicements

Scarring alopecia can be a reason of permanent hair loss and damage on scalp. So, cure of scarring alopecia should be a little bit aggressive. The nature of cures varies depending on the special diagnosis. Scarring alopecias that involve generally lymphocyte inflammation of hair follicles, such as lichen planopilaris and pseudopelade, are generally examined with corticosteroids in topical creams and by injection through the affected area.


Once a scarring alopecia has reached the burnt-out part and there has been no more hair fall for a lot of years, bald parts can be either surgically removed if they are not too big or the bald patches can be transplanted with hair follicles taken from unaffected parts
